Work Schedule: Full-Time benefit eligible 24 hrs./wk., 8am-noon, M-F, semi flexible hours with additional evening and weekends hours, as needed.
Supports the growth, sustainability and measurable impact of the Caring for Our Communities initiative. Provides specialized support in data tracking, grant writing, grant reporting, program documentation and outcome reporting to help demonstrate the value of community health work across Methodist Jennie Edmundson Hospital, the Jennie Edmundson Foundation and community partners.
Responsibilities:
Essential Functions
Data Collection, Tracking, and Reporting (50%)
Collect, organize, and maintain program data related to referrals, services provided, flexible funding supports, Community Health Coach activity, partner engagement, and program outcomes.
Support data tracking through systems such as Unite Us, internal spreadsheets, hospital reports, partner updates, and other approved tools.
Prepare regular data summaries for Community Health leadership, hospital leadership, board committees, donors, and external funders.
Compile data for ROI analysis, grant reporting, community benefit reporting, program evaluation, and strategic planning.
Review data for accuracy, consistency, and completeness, and help identify gaps, trends, and follow-up needs.
Grant Writing and Funding Support (30%)
Draft, edit, and prepare grant proposals, letters of inquiry, funding requests, and supporting documents for Caring for Our Communities and related community health programs.
Gather program information, budgets, partner details, outcome data, community impact examples, and other documentation needed for grant submissions and reports.
