The Health Home Reporting & Data Specialist is a remote, systems-focused role that supports the success of Health Home services through advanced data analysis, reporting, and operational insight. This position does not provide direct or in-home care, but instead serves as a key technical resource—ensuring data accuracy, identifying trends, and supporting informed decision-making that improves care coordination and program performance. Through collaboration and continuous improvement, this role helps strengthen the quality, efficiency, and impact of services across the organization.

Key Responsibilities

Maintain and manage healthcare tracking systems to ensure accurate, up-to-date data for care coordination teams

Cross-reference multiple systems to validate data accuracy, completeness, and workflow alignment

Analyze large and complex data sets to identify trends, gaps, and opportunities for improvement

Develop reports, dashboards, and data visualizations to support leadership decision-making and program performance

Support auditing processes and conduct detailed reviews to ensure data integrity and compliance

Investigate and resolve data discrepancies using strong analytical and problem-solving skills

Provide timely, accurate reporting to support program requirements and regulatory standards

Collaborate with internal teams to support effective communication and coordination through data sharing

Develop expertise in Health Home systems and provide guidance and support to team members

Contribute to process improvement efforts, special projects, and operational enhancements
Requirements, Skills, Knowledge and Expertise

High School/GED required

Associate's/Bachelor's Degree preferred
WORK EXPERIENCE/TRAINING/ADDITIONAL REQUIREMENTS:

Prior experience in care coordination, administrative healthcare roles, or related support function
